This form is an alternative to writing a letter of complaint or filing your complaint by phone. Letters and phone calls are the preferred method of filing a complaint.

This form shall be completed for all filings of complaints regarding issues pertaining to building, property maintenance or zoning codes ONLY. The person or persons filing the complaint shall understand that by signing and/or attesting to the information contained herein may be asked and/or summoned by subpoena to provide testimony in a court of law.
